Patient Portal @ MMH

Have you signed up for Montrose Memorial Hospital’s Patient Portal yet?  Do you know what it is?  Our Patient Portal is a secure online website that gives MMH patients convenient 24-hour access to your personal health information from anywhere with an Internet connection. After you have registered for the Patient Portal, you will be able to log in with your username and password and view health information from your hospital visits.  This will include your health summary, visit history, allergies and conditions and upcoming hospital appointments.  You can access the portal on your schedule, whether at home, on vacation, or at another medical office.  The information on the Patient Portal comes from your MMH Electronic Health Record and will not include office records from your physician.  

To sign up you can visit our website at or stop by the hospital and we are happy to help you enroll.  We have a Patient Portal/Medical Records staff member located in the lobby area by Registration from 9-4 Monday through Friday who will walk you through the steps.  If you choose to register online, you must have a current e-mail address on file with the hospital and your medical record number to begin the process.

Many people have asked, “Is my information safe?”  The answer is yes!  Patient Portal passwords are encrypted and URLs are re-written so that they cannot be copied and pasted.  You are the only one who can access your Portal.  We also have a timeout feature that protects your information if you leave the Portal page open.

If you would like, you can also give family members access to your Portal.  If you would like your parents, children or health care proxy to be able to see your hospital information, you must fill out the Proxy Request form at Montrose Memorial Hospital which requires a signed consent from you.  You will need to come into the hospital to receive this form from our Medical Records department.
