Nutrition Services

Inpatient Nutrition and Diabetes Services

The goal of inpatient nutrition services is to assist patients who are malnourished or at risk to become malnourished develop strategies to optimize their intake. This can include

  • changes in diet
  • addition of nutrition supplements
  • specialized nutrition support such as a feeding tube or parenteral nutrition. 

Meeting your nutritional needs helps to decrease problems with wound healing, incidence of infection and overall mortality and morbidity

Our dietitians provide nutrition counseling to address a variety of medical conditions and disease states such as heart disease, diabetes, renal disease, gastrointestinal problems, and cancer. This can include

  • Counseling  you about your special diet
  • Helping you to set goals to improve your health.
  • Showing you how to use a glucose monitor

Outpatient Nutrition and Diabetes Services

Our outpatient dietitians provide nutrition counseling on diets for a wide variety of medical conditions and disease states. During your appointment you will receive individual counseling and specific guidance on your diet. Your registered dietitian will:

  • Take your diet history and help you set goals that fit your lifestyle. 
  • Analyze your usual food intake and suggest changes to improve your eating habits. 
  • Provide a personalized diet instruction plan based on mutually determined goals. 
  • Inform your primary physician regarding your nutritional care plan.

Diabetes Classes

Diabetes Self Management Classes are offered periodically throughout the year. This is provided as a series of 5 - 2 hour classes with education and self management training on all aspects of diabetes care. Our diabetes program is recognized by the American Diabetes Association as meeting the national standards for quality education. Medicare and insurance companies may pay for all or part of your education. Please check with your provider. These classes require a referral from your healthcare provider. Please ask them to fax a referral to (970) 252-2767.

  • Heart disease / high cholesterol
  • Nutrition in pregnancy
  • Weight management ( to gain or lose )
  • Eating disorders
  • Pediatric nutrition issues (FTT/ feeding tubes, weight)
  • Sports nutrition
  • Bariatric surgery
  • Celiac disease
  • Gastrointestional issues (diarrhea, Crohn’s, IBS/IBD)
  • Other

  • Contact your physician and ask them to fax us a referral (970) 252-2767.
  • We will contact you to schedule an appointment.
  • Check with your insurance company to determine if they will cover your visit for “outpatient medical nutrition therapy”.
  • Additional appointments may be recommended after your initial visit to assist you in your lifestyle change and to provide additional information.
